Home Care Packages (HCP) are services designed specifically so that you can remain living at home with the supports you require. We provide support for people with a higher level of care needs that require care coordination and case management to help you live independently. These packages of care provide choice and flexibility in the way your care is provided which might include who delivers your supports, how they will be provided and when they are delivered. This process is called Consumer Directed Care (CDC).
Home Care Packages
Home Care Package Services include but are not limited to
Support services – such as washing and ironing, house cleaning, gardening, basic home maintenance, home modifications related to your care needs and transport to help you with shopping, visit your doctor or attend social activities
Personal care – such as help with showering or bathing, dressing and mobility
Nursing, allied health and other clinical services – hearing and vision services
Care coordination and case management
Levels of Care
There are four levels of Home Care Packages which can give you the care and services you need:
- Level 1 – supports people with basic care needs
- Level 2 – supports people with low level care needs
- Level 3 – supports people with intermediate care needs
- Level 4 – supports people with high level care needs
Each package level is funded at different amount of funding subsidy that is paid by the Australian Government to a service provider like Neighbourcare that delivers care and services to you. The package support level is assessed by the Aged Care Assessment Team (ACAT).

From February 2017 people eligible for a Home Care Package are able to select a service provider of your choice to deliver services. Neighbourcare will be happy to discuss your needs with you.
